Enrollment Information

Prospective students interested in enrolling in one of our CNA/GNA programs can do so by coming in person to register during our business hours. Appointments are not required to register and therefore you can walk-in. We are open Monday-Friday from 8:30 am-5:00 pm.

You can also download, print and complete the required forms. After completion, the forms can be brought along with the other requirements the first day of class.

Requirements for Enrollment

For Traditional CNA/GNA Program:

Registration Requirements 1 – 6 have to be met by the time of registration.

Registration requirements 7 – 11 have to be met by the first day of class.

  1. Must be at least 18 years of age
  2. Bring your photo ID and your social security card
  3. Complete our enrollment application. You can also download, print and complete enrollment application. Please bring the application with you at the time of registration.
  4. Bring your high school diploma, college diploma or GED
  5. Take the Wonderlic Basic Skills Test, a test that evaluates basic English skills.
  6. Bring your $100.00 registration fee. Payments can be made by credit card (VISA, Mastercard, and American Express), cashier check or money order.
  7. Acceptable Background check. Click here for the background check form and instructions.
  8. CPR certification from American Heart Association or American Red Cross. A certified American Heart Instructor will offer a class during the course of the program for students who do not have their CPR/First Aid Certification. The cost of the 5-hour class is $60.00.
  9. Proof of PPD (Tuberculosis) test within 9 months of the enrollment date. A positive PPD needs to be confirmed with a chest x-ray.
  10. Proof of Hepatitis B vaccine or signed waiver form. Click here for the Hepatitis B waiver form.
  11. Submit a copy of a current health physical examination that was completed within 9 months of enrollment date. Click here for the school physical form.

Please note that you are not required to have a high school diploma to register for the traditional CNA/GNA training program.

Enrollment Requirements for the GNA Certification Training

Registration Requirements 1 – 7 have to be met by the time of registration.

Registration requirements 8 – 12 have to be met by the first day of class.

  1. Active Certified Nursing Assistant license in good standing with the MBON
  2. Must be at least 18 years of age
  3. Bring your photo ID and your social security card
  4. Complete our enrollment application. You can also download, print and complete enrollment application. Please bring the application with you at the time of registration.
  5. Bring your high school diploma, college diploma or GED
  6. Take the Wonderlic Basic Skills Test, a test that evaluates basic English skills.
